Before Your First Appointment
Welcome to the Sleep and Alertness Clinic!
Please complete our questionnaire to assist us in assessing your current sleep concerns, save and email your completed copy to nicole@sleepalert.ca. You may also attach the completed file and respond directly to the staff member that emailed you the questionnaire.
Your completed questionnaire is an important part of the referral process and is required before we can proceed with booking your first appointment. Note that a majority of our appointments are in person, but please refer to your latest email instructions for details.
Once we have received and reviewed the questionnaire, we will contact you regarding appointment scheduling and any additional info that may be needed.

If you would prefer to print a paper copy and fill it out by hand, you are welcome to do so. You may scan and email the completed copy to nicole@sleepalert.ca or bring it with you to your appointment.

“Why is this questionnaire so important?”
Disrupted or disordered sleep can be associated with medical conditions, mental health disorders, medication use, substance abuse and a variety of other factors. It is imperative that we get the complete picture so that we can give you the best care.
Please give yourself ample time to complete the questionnaire. The questionnaire is very important as it will help us to assist your current sleep concerns.

The address of the Sleep and Alertness Clinic (SAC) is:

The closest major intersections to SAC is Church and Richmond.
The closest subway station to SAC is Queen Station.
Please be aware that there is a 10-step staircase leading to our clinic entrance, which may be difficult for patients with mobility issues, so please let us know if you have any concerns.

We are on the main floor of the Davpart Building on 4576 Yonge Street in North York. Once you enter the front doors, turn left towards the glass double doors.
